Haley House Admission Criteria

| The admission criteria are
based on the Fisher House criteria, as the Haley House is an interim program
until the Fisher House is completed. |

|
|
| Family members are
defined as an important person to the veteran or active duty service member
who he/she believes as being their primary support system (e.g. family
member can be a fiancė, girlfriend/boyfriend, neighbor, etc.). The
intent is to provide temporary lodging to the individual of the
veteran/active duty soldier who will be able to provide comfort to them in
their time of need. An additional room can be authorized if the
support system is such that 2 rooms are needed (e.g. divorced parents of a
young service member). |

| ADMISSION CRITERIA |

Family members of
veterans or service members |
|
| |
|
- Whose hospitalized loved one is seriously ill,
undergoing surgery, receiving rehabilitation, hospice or any other
significant medical condition, and
- Who live over 50 miles or 1 hour from the
hospital, and
- Have experienced a financial hardship and have no
other means to pay for temporary lodging (funds for the Haley House are to
be used as a "payor of last resort", and
- Who are medically stable, independent with their
own care, and capable of unsupervised setting (unless they bring their own
caregiver), and
- Who need no more than up to 2 weeks of temporary
lodging with some exceptions due to extended rehabilitation stays, and
- Who express a willingness to respect the rights
of other guests and to contribute to a supportive and caring environment,
and
- Agree to an assessment with the primary care
social worker to determine financial hardship.

- Who is undergoing extended episodic outpatient therapy, treatment or
evaluation, and
- Who is accompanied by a spouse or caretaker but is not limited in
activities of daily living and medically stable.
